Investigation Summary
At 10:00 a.m. on March 6, 2019, an employee was freeing a metal piece within the machine and had removed the plexi-glass guard to rest the sensor. The employee reached into the machine and asked his coworker to start the machine to see if he could pull out the remaining material from the machine. The employee caught three fingers from his left hand within the rollers, amputating one and severely lacerating the other two. The employee was hospitalized.
