Employee is struck by bar screen and incurs multiple injurie

At 1:00 p.m. on February 23, 2019, Employee #1 was in a confined space as a 1,200 pound steel bar screen that was being lifted by an overhead crane. The sling holding the bar screen failed, causing it to fall and strike the employee. The employee suffered a concussion, broken ribs, and back injuries. He was hospitalized for several days.
