Investigation Summary
At approximately 10:15 p.m. on March 5, 2019, an employee, who was part of the sanitation crew, was cleaning the gizzard peeler machine. The employee did not lock out the machine. The employee had manipulated the safety switch that stopped the peeler rollers from turning. When employee tried to remove some meat from the rollers, they powered on catching her fingers. The employee amputated the first digit from right middle and ring fingers and nail loss on the index finger. The employee was hospitalized.
