At 5:15 p.m. on January 23, 2019, Employee #1 was standing on a cable spooler holding on to the chain when the chain slipped and he jumped off and fell 3.5 to 4 feet. He fractured the wrist, the right pelvis, and ruptured the urethra. The employee was hospitalized.
