Employee amputates tip of finger while replacing roller on c

At 8:30 p.m. on February 28, 2019, Employee #1 was replacing a roller on conveyor wile attempting to guide the shaft bearing into position on the conveyor. The roller shaft was being lifted by an overhead hoist using remote controls. This hoist was being operated by the employee. Just prior to the injury, the employee had his finger in the shaft hole to guide the hex head shaft into place. The employee bumped the remote opening controls of the overhead hoist, causing the shaft and roller to move amputating the finger tip. The employee was hospitalized.
