Investigation Summary
At 6:15 p.m. on February 21, 2019, an employee was operating a rolling mill line. During operation, the employee placed the equipment in jog mode, which eliminates the light curtain protection of the mill. He did this while simultaneously placing his left hand inside the machine. The employee's glove got caught by some perforated metal, pulling his hand into a pinch point. As a result, the fingertip of his left index finger was amputated. The employee was hospitalized.
