Investigation Summary
At 1:15 p.m. on January 22, 2019, an employee was preparing wire for a welded cross over on machine when he dropped the wire onto a running spool. The employee reached in to grab the wire and caught his hand against the wall of a guide cone. The employee amputated a finger and was hospitalized.
