Investigation Summary
At 8:15 a.m. on January 10, 2019, Employee #1 was holding the hose of filling station #3, which was connected to the bean bag. The blower machine of the filling station gets filled with large quantities of foam material. The employee put his left hand in to unclog/unjam the blower (3455rpm) and his left hand suffered from injury. The employee was hospitalized with fractured fingers and bone loss in one of the fingers of his left hand.
