Investigation Summary
At 10:20 a.m. on February 13, 2019, Employee #1 was in the warehouse area of the retail store where merchandise was being put away by hand by several employees and with a Crown Stacker lift. The accident occurred when an 85 pound box of merchandise fell approximately 15 feet from a merchandise rack and struck the employee as she was working below. The box was pushed off of the rack when another coworker was using a crown stacker lift to put away a pallet of merchandise, which pushed the 85 pound box off of the rack. The employee was admitted and treated at the hospital for two days due to injuries to her head, neck and spine.
