Investigation Summary
At 3:45 p.m. on January 4, 2019, Employee #1, a finger jointing operator, was performing a change-over on the Industrial finger joint machine center. He turned the machine off, and adjusted the throat to the carriage on the back side of the finger joint machine center at the unguarded fly saw. After the adjustment was completed, the employee had a coworker turn the finger joint machine center back on. As the employee was leaving the area, he slipped. His left hand came in contact with the rollers on the shuttle of the fly saw, crushing his finger. Employee #1 was taken to a nearby emergency center. He was transferred to a larger facility for further treatment, where he was hospitalized overnight.
