Investigation Summary
At 3:30 a.m. on February 1, 2019, an employee was operating a printing line C52 at a commercial printing facility when the printing web broke. The employee was webbing up the line when it jammed the Goss folder machine and he tried to clear the jam that was in the folder while the line was running. The employee caught and amputated his left index finger between the gripper drum and the diverter plate. The employee was hospitalized.
