Investigation Summary
At 2:15 p.m. on November 14, 2018, an employee was burning weeds with a weed burner or a drip torch that was mixed with gas and diesel. The employee sustained third degree burns over a large percentage of his body when the wick fell out and the flames entered the tank, exploding the weed burner. The employee was transported to the hospital and transferred to Liberty of Utah, where he was admitted for treatment within their intensive care unit.
