Investigation Summary
At 1:45 p.m. on January 18, 2019, an employee was cleaning the throat and screw on a plastic extrusion machine. Before cleaning, the employee depressed a button to stop the machine, but inadvertently activated the machine instead. As the employee reached into the throat of the extruder with a rag in hand the rotating screw of the extruder pulled the rag and the employee's hand into its moving parts. As a result, the employee's right middle and index fingers were amputated. The employee was hospitalized.
