Investigation Summary
At 2:00 a.m. on January 28, 2019, and employee was cleaning the screw conveyor while the machine was in motion. The employee was using a rag which was caught in the conveyor and pulled her right hand into the machine. The employee was able to pull the emergency cord on the machine and several coworkers came to assist her. The machine had to be disassembled before the employee could be extricated and she amputated her pinky from her hand and sustained severe damage to her ring and middle fingers. The employee was transported to the Regional Medical Center, where she was hospitalized.
