Investigation Summary
At 8:30 a.m. on December 15, 2018, Employee #1 was helping a coworker, a caster operator, open lance Line #1 and Line #2 to the tundish on the west side of a commercial metals shop. Employee #1 helped the coworker divert the hose to Line #1 and went to hook up the hose to prevent a tripping hazard. Employee #1's glove caught on fire when Line #1 opened and hit the table causing sparks to spread in the area as he grabbed the lance. Employee #1 sustained 2nd and 3rd degree burns. He was not hospitalized for injuries.
