Investigation Summary
At 12:15 p.m. on January 14, 2019, an employee was performing preventative maintenance of Conveyor #6 which was part of a plastic injection molding machine. The employee plugged the motor into the wall outlet before greasing it and put some grease onto his fingertip and turned the motor on before placing his left hand onto the conveyor. The employee caught his index finger in the moving conveyor chain and amputated it. The employee was hospitalized.
