Investigation Summary
At 7:34 a.m. on January 14, 2019, an employee was setting up a Repak RE25 meat packaging machine. The employee's hand was in the point of operation at a three foot opening that was not guarded on the right side of the machine when his coworker, a mechanic, turned the machine on from the control panel on the left side of the machine. The interlock was not operational to prevent the machine from coming on when the guard was removed. The employee suffered an amputation of his left index finger. The employee was hospitalized.
