Investigation Summary
At 7:40 a.m. on November 7, 2018, an employee was troubleshooting a ball valve for the auto sampler of a protein bulk tote line that was not pulling a sample. The employee placed his finger into the energized ball valve to see if it was in the open position. The valve cycled and the employee sustained a partial finger amputation when it was caught in the valve.
