Investigation Summary
At 9:15 a.m. on November 19, 2018, an employee was climbing a 16 foot Westward extension ladder to repair an electrical box on an overhead crane in the maintenance area. The employee fell approximately 5 feet from the ladder when it slid out from underneath him, fracturing and lacerating his left hip. The employee was hospitalized.
