Investigation Summary
At 12:15 p.m. on December 21, 2018, an employee was cleaning an extruder machine. The employee had finished the production for the day and started cleaning the inside of the extruder machine with a rag while the machine was powered and in motion. During the cleaning process, the employee did not de-energize the equipment. The employee's left hand was caught by moving parts of the machine. The employee was hospitalized as a result of compound fractures and partial amputation of the left index, middle and ring fingers.
