Investigation Summary
At 6:46 a.m. on December 26, 2018, Employee # 1, an RCL lid line machine operator, cleaned the set of three horizontal polish rolls on Line 7 in preparation for starting up the line. This constituted a normal weekly task. The procedure for cleaning the polish rolls calls for opening the rolls, which creates a gap to eliminate the in-going nip points between rolls; engaging the blocking mechanisms on both sides of each roll to prevent the rolls from closing; and locking out the air to the traverse. Then, the operator uses a rag to manually wipe down the rolls with the rolls not running/rotating. At the time of this incident, however, Employee # 1 did not have the blocking mechanisms engaged and had the rolls running while wiping the rolls with a rag. One end of a roll moved down, closing the gap between rolls, and the operator's rag and right arm got pulled into the in-going nip point. Employee # 1 suffered a wound with no fractures to the right arm that required hospitalization, which included 48-hour antibiotic administration and exploratory surgery.
