Investigation Summary
At 11:45 a.m. on January 2, 2019, Employee #1 was operating a mechanical press to cut metal. The employee's fingers went through the guard, to the clamp that hold the materials in place. He pressed the foot switch and felt the clamp on his finger and pulled back. The employee initially suffered a severe laceration on his left index finger. The tip of the finger later had to be amputated.
