Investigation Summary
At 11:00 a.m. on November 26, 2018, an employee was replacing a valve on ceiling mounted heating system and had climbed onto the forklift tines. The employee fell approximately 20 feet after losing his balance and sustained lacerations, contusions, and multiple fractures. The employee also punctured his lung and was hospitalized.
