Investigation Summary
At 1:36 p.m. on December 12, 2018, Employee #1 was operating a forklift. The employee was doing a "let down" when he lost control of the equipment and was pinned between the storage rack and reach fork. He placed a pallet on the aisle floor and went to get another pallet to fulfill the order, when he forgot that he placed one pallet on the aisle floor. The employee suffered injuries to his lower back, strain/sprain. The employee was treated at Dameron Hospital in Stockton CA.
