Investigation Summary
At 5:45 a.m. on November 30, 2018, an employee operating a press were she perfor ated approximately 80 pieces. The employee's manager then assigned the employee to to operate the AVIR Press-Rite, (Model: 1A, Serial Number: 8462), where she was going to bend approximately 500 pieces of small metal laminate. The employee used her right hand to place the part on the point of operation, activate the p ress via a foot pedal and remove the part with her left hand. At 7:40 a.m., the employee had completed 15 pieces when her right middle finger was caught at the point of operation. The employee sustained a finger amputation injury when she c ame in contact with the machine's point of operation that was not guarded. The e mployee was transported to Hand and Orthopedic Center of Southern California, wh ere she was treated for her injury.
