Investigation Summary
At 5:45 a.m. on November 30, 2018, an employee operating a press were she perforated approximately 80 pieces. The employee's manager then assigned the employee to to operate the AVIR Press-Rite, (Model: 1A, Serial Number: 8462), where she was going to bend approximately 500 pieces of small metal laminate. The employee used her right hand to place the part on the point of operation, activate the press via a foot pedal and remove the part with her left hand. At 7:40 a.m., the employee had completed 15 pieces when her right middle finger was caught at the point of operation. The employee sustained a finger amputation injury when she came in contact with the machine's point of operation that was not guarded. The employee was transported to Hand and Orthopedic Center of Southern California, where she was treated for her injury.
