Investigation Summary
At 12:45 p.m. on December 8, 2018, an employee was installing a conductor casing for a surface well and the chains that were being used to hoist the apparatus was not balanced properly. The employee pinched his left hand's index finger between the chain and conductor casing when the chain shifted and partially amputated his finger. Hospitalization was not required.
