Investigation Summary
At 6:30 a.m. on November 15, 2018, an employee was unloading rolls of sheet metal from a flatbed truck. Each roll weighed approximately 8,000 pounds. During unloading, the employee released a strap on one of the sheet metal rolls. The roll fell and crushed his foot. The employee was hospitalized and received treatment for his injury, resulting in the amputation of his lower leg.
