Investigation Summary
At 6:45 a.m. on November 28, 2018, Employee #1 was using a saw. The saw stalled so the employee went underneath the mill to back up the belt by hand. The employee was manually turning the belt on the drive to reverse the drive belt when another coworker started the machine. The employee's right index finger tip was amputated. The employee was hospitalized.
