Investigation Summary
At 10:40 a.m. on November 28, 2018, Employee #1 was operating a Roto-Finish Model ST-20BCPT tumbling machine to deburr metal parts. A piece of stone media became jammed between the gate and the exit chute. Employee #1 reached in the opening and removed the stone without deenergizing the machine including the pneumatic pressure on the gate, resulting in the gate closing on the tip of his right middle finger. Employee #1 suffered a partial amputation to the tip of the right middle finger. The employee was hospitalized.
