Investigation Summary
At 8:10 a.m. on November 17, 2018, Employee #1 was loading a roll of paper into a corrugated modular facer machine. The employee inadvertently used the incorrect control switch, which returned the roll of paper towards the millstand arms, striking him from behind, pinning him between the paper roll and arm. The employee sustained minor fractures to L6, L7 and L8 of lumbar region. The employee was hospitalized.
