Investigation Summary
At approximately 2:00 p.m. on November 7, 2018, Employee # 1 was cleaning the exterior of the Twister T2 Trimming Machine. She reached over the tumbler of the machine when her right hand was caught between the tumbler and the frame of the machine. Employee # 1's right hand was lacerated and her index finger was amputated. She did not lock out tag out before cleaning the unguarded machine. She was hospitalized.
