Investigation Summary
At 10-:30 a.m. on October 22, 2018, an employee was checking on the operation of a portable conveyor because he thought that it was leaking oil onto the floater silo. The employee stepped onto an opening on the side of the floater and it slid into the machine. The employee's left big toe was struck by the conveyor and amputated the toe. The employee was hospitalized.
