Investigation Summary
At 7:39 a.m. on November 12, 2018, an employee was working to disassemble a scaffold alone. The employee was working to remove the third level ladder portion of the scaffold, shaking it to break it loose. At some point, the employee lost balance and fell approximately eighteen feet and received fractures to his left heel, right ankle and compression injuries to three spinal discs.
