Investigation Summary
At 12:30 p.m. on November 16, 2018, an employee was rip cutting a board on an arbor table saw using a push stick. The employee's left hand was drawn into the saw blade when the board kicked back. The employee amputated his left thumb and partially amputated his index and middle fingers. The employee was hospitalized.
