Investigation Summary
At 5:30 a.m. on November 8, 2018, an employee was working with two coworkers while troubleshooting how to get a wheel out of clamp. The employee got the clamp to open slightly and lifted the wheel out. One of the coworkers replaced a limit switch and pulled a cord that shut off the air at a valve while the employee noticed excess grease on the clamp and wiped it off with his right hand. The employee caught his left hand's pinky fingertip between two parts of the clamp after his coworker had restarted the air flowing and amputated it. The employee was hospitalized.
