Investigation Summary

At 12:00 p.m. (noon) on November 6, 2018, an employee was working at an elementary school. She was employed as an Intensive Behavior Instructional Aid by a school district in California. Her immediate supervisor was not present at the time of the incident. She was standing and watching students in the playground area. She was suddenly pushed from behind by a special needs student. She fell onto the ground. She sustained a serious injury. Her diagnosis for compression of discs. She was hospitalized at Hemet Valley Hospital in Hemet and released after three days. An employer's representative reported the incident to the San Bernardino District Office at 4:38 p.m. that same day. The employer did meet the Division's reporting requirements. A CSHO from San Bernardino District Office arrived and opened an inspection at approximately 12:15 p.m. on November 16, 2018. The employee had received training on how to identify, calm, and restrain (if necessary) an aggressive student.
