Investigation Summary
At 3:15 p.m. on October 25, 2018, Employee #1 identified a quality issue with one of the four ink rollers on the SOS Press Machine, Number 142. Per normal practice, the employee opened the guard for the ink roller and proceeded to use a wire brush and cleaning solution to clean the surfaces of the ink rollers while the machine was running in normal production mode and speed. The employee fell towards the unguarded ink roller, and tried to catch herself with her right hand. Her right hand became caught in the ingoing nip point of the ink rollers, resulting in a the partial degloving of the right hand. Employee #1 was hospitalized for treatment.
