Investigation Summary
At 10:15 a.m. on October 17, 2018, Employee #1 was cleaning up the line during a changeover. While changing over from milk to dark chocolate, the line operator cleared out the majority of the chocolate in the tempering kettle. The coworker left the line to turn in some paperwork while Employee #1 noticed some chocolate that still needed to be cleaned from the lower opening on the kettle. When she reached in to clean it, the paddle swept by the hole her finger was reaching into causing an amputation of the fingertip on the right hand. The employee was hospitalized.
