Investigation Summary
At 2:05 p.m. on October 15, 2018, an employee was riding in a safety cage that was lifted by a forklift to assist with moving a tarp to cover a pile of merchandise. The safety cage was not secured to the forklift and, when it was lifted, it slid off of the forks. Emergency Services responded and the employee sustained a compound fracture to his left arm just able the elbow and multiple fractures to his left shoulder. The employee was hospitalized.
