Investigation Summary
At 3:00 p.m. on October 12, 2018, an employee was operating a pinch press and flipped a two hand trip switch to cycle the machine when asked by a coworker. The employee did this five times and then placed his hands in the machine. The employee partially crushed both hands when the pinch press cycled and came down on his hands, puncturing them. The employee was hospitalized and sustained nerve and tendon damage in both hands.
