Investigation Summary
At 1:15 a.m. on September 25, 2018, an employee was working in the picking area that was adjacent to the dock area where a coworker was picking up empty racks with a forklift. The employee had his foot caught by the forklift when his coworker started to back it up and ran over the employee's foot. The employee amputated his second and third toes from his left foot as well as sustaining a fracture. The employee was hospitalized.
