Employee fractures forearm in fall from equipment

At 2:15 a.m. on September 8, 2018, Employee #1 was working the night shift operating the nut bankout shuttle. The employee backed up the equipment and was trying to lower the bucket but mistakenly pulled the wrong lever to activate the rear steering. When she pulled the lever, the equipment jerked causing her to fall off the seat onto the dirt ground fracturing her forearm. The employee was hospitalized for treatment.
