Investigation Summary
At 10:00 p.m. on August 31, 2018, an employee was making a production changeover in the bakery and had not removed the dough from the production machine. The remaining dough along with the paper became hammed at the cutter blade and the employee tried to pull it out from the operator's side rather than both sides. The employee's right hand was underneath the cutter when it came down and amputated several fingers. The employee was hospitalized.
