Investigation Summary
At 8:15 a.m. on September 7, 2018, an employee was unloading bridge diaphragms stacked on wooden four inch by four inch boards on a semi-trailer. One of the boards was broken which left the stacked diaphragms unstable and they fell over as the employee unhooked them. The employee tried to jump off of the stack, slipped and fell into another stack of diaphragms. The employee received rib fractures in the fall.
