Investigation Summary
At 2:38 p.m. on September 13, 2018, an employee was unloading a truck and placing boxes on a wooden pallet when the forklift that was holding the boxes lunged forward and pinned the his leg against the truck. The employee crushed and fractured his leg. The employee was hospitalized.
