Investigation Summary
At 9:45 a.m. on August 16, 2018, Employee #1, employed by a scrap yard, was working to clean a tail pulley on the cyclone conveyor. The cyclone conveyor was not locked and tagged out. Employee #1 loosened the tensioner to slacken the belt and climbed in to the belt to get a better angle for cleaning. As he was cleaning the conveyor, a coworker, the shredder operator, started the conveyor. The coworker was unable to see that Employee #1 was in the conveyor. Employee #1's arm and leg was rotated into the bottom side of the tail pulley. After being pulled into the pulley, a detection system built into the belt shut off the conveyor. Employee #1 was able to exit the conveyor. A horn alarm sounded 30 seconds prior to the belt starting to signal an all clear. Employee #1 thought that they were not running material that used this conveyor so continued cleaning. He received lacerations and puncture wounds on his hands. His left hand also had a few broken fingers. He was hospitalized.
