Investigation Summary
At 12:20 p.m. on August 18, 2018, an employee was performing maintenance on Palletizer #7 and missed a step in the lockout/tagout procedures, lowering the "head" or "platen" down to the floor prior to releasing the stored energy. The employee pinched his left hand's index finger in the chain and sprocket when the platen fell to the floor. The employee sustained an avulsion to his finger. Hospitalization was not required.
