Investigation Summary
At 6:30 a.m. on August 24, 2018, an employee was trying to unplug an airlock on an air lift by blowing compressed air through it to move a pipe. The employee r estarted the airlift and put his hand into the lower section of it to pull the p ipe out. The employee reached too far into the machine and amputated the tips o f both his index and middle fingers. The employee was hospitalized.
