Investigation Summary
At 1:45 p.m. on August 20, 2018, an employee was working with two coworkers to weld a repair to a saw mill's carriage log turner. The employee was using a 2 inch by 4 inch board to pry a portion of the machine to hold the stack that holds all of the equipment in place. The employee was unaware of another lever arm that cycled simultaneously and had his arm caught between the lever as it cycled and a fixed portion of the equipment. The employee fractured his arm in two places and was hospitalized.
