Investigation Summary
At 4:15 a.m. on August 15, 2018, an employee was changing the ram head of the Eitel Stroke Controlled Straightening Press #1 and had not locked it out. The employee was holding the ram head with her left hand while bringing down the ram of the press by pressing the level with her right hand. The employee pinched her left thumb between the ram and the ram head, crushing the finger. The employee sustained a partial amputation of her finger later. The employee was hospitalized.
